Medicaid expansion is first and foremost a way to send more funding into rural and urban hospitals that have poorer customer bases. It's the same as food stamps where the money ends up not in the hands of the poor but in the hands of the grocer. The money comes from the taxes that are raised from economic healthier areas since the taxes raised locally would not touch the cost.
